Important information:
You must let the nursery manager know if you are on any waiting list at any other nursery as you may be charged if you are claiming elsewhere and go over your funding entitlement.
You must let the nursery manager know if your child is being supported by other professional e.g. social workers, speech and language therapist, special educational needs professionals. This is so that we can best support your child’s development.
I confirm that all information provided is correct and agree to inform the nursery of any changes. I agree that if I fail to disclose any information, I may lose my child’s place at the nursery.
Agreements and Consent Form
I agree that, if I fail to inform the nursery of my child’s absences and if I cannot provide a good reason for my child’s non-attendance, it may result in my child losing their place. If your child receives funding, the nursery will have the right to terminate your agreement with the council if your child does not attend regularly in a term without good reason and/or non-attendance amounts to more than 4 weeks in any one term of funding or 10 consecutive days of unauthorised absences.
I agree to provide the nursery with nappies, baby wipes and sealed formula milk for bottle feeding babies.
Permissions
I/We hereby give permission for Fun Time Nursery to take photos and videos of my/our child to use within the nursery for displays, art work, EyLog observations etc.
I/We hereby give permission for Fun Time Nursery to apply sun cream to my/our child.
Sun Cream is to be provided by parent(s)/carer.
I/We hereby give permission for Fun Time Nursery to apply nappy cream to my/our child if necessary.
Nappy Cream is to be provided by parent(s)/carer.
I/We hereby give permission for Fun Time Nursery to administer prescribed medication or Calpol.
Any medication to be given to the child is to be provided by parent(s)/carer.
I/We hereby give permission for Fun Time Nursery to apply a plaster on my/our child, if they have cut or grazed themselves and if necessary.
I/We hereby give permission for Fun Time Nursery to take my/our child off the nursery premises to go for walks, go to the shops, go to the park, go to the library etc.
I/We hereby give permission for Fun Time Nursery to contact my child’s playgroup or school to discuss their developmental progress. This will be undertaken to assist my child to reach their full potential.
I/We hereby give permission for Fun Time Nursery to allow my/our child access to the nursery’s computer and internet facilities, with supervision from a nursery practitioner. I understand that all internet sites will be suitable for the age range of the children in the room.
I/We hereby give permission for Fun Time Nursery to use hand sanitiser on my child for the purpose of lowering the risk of COVID-19. (This will be accompanied with hand washing throughout the day)
I confirm that the information given above is correct and will inform of any changes as soon as possible to the Nursery Manager.
Admission
To secure a place for your child you will be required to, fully complete and return the nursery Registration Form; FEEE Form (for those receiving Free Early Years Educational Entitlement); and bring in a copy of your child’s birth certificate or Passport.
Fee Payment:
Fees must be paid one month in advance by the first day of the month.
Payment is required by Standing Order, Internet/Telephone banking. Details of Fun Time Nursery Bank account are available on request for internet and telephone banking payments
A £15 administration fee will be charged on a weekly basis for all late payments.
Opening times
The nursery sessions run from 9am to 4pm Monday to Friday. The nursery is only open during the school term times (38 Week per year) – details of the school term times can be found at: www.derby.gov.uk/education-and-learning/schools-and-colleges/term-and-holiday-dates. The nursery is entitled inset days, when the nursery will be closed – the nursery will inform parents when the nursery is to remain closed.
Termination, cancellation and change of sessions
One-month notice is required for any change of sessions or termination of agreement. If parents choose to leave prior to the end of their notice, fees are non-refundable. The minimum period for any permanent change of session is one month.
The nursery reserves the right to terminate the agreement with immediate effect in cases of non-payment of fees; or if a parent, carer or child displays abusive, threatening or otherwise inappropriate behaviour; or for any other reasonable cause. Intimidation or abuse of our staff will not be tolerated and may result in immediate termination.
Insurance
The nursery has extensive insurance cover for nursery-based activities and outings. Details of the insurance may be requested from the nursery manager. The certificate is displayed in the nursery.
Personal property and belongings
The nursery cannot be held responsible for any loss or damage to any parent’s, carer’s or child’s property or belongings. Every reasonable effort will be made by the nursery staff to ensure that property or belongings of any parent, carer or child is not damaged. Please ensure your child’s clothing is clearly labelled and we suggest that all toys, books and equipment are left at home.
Parents should supply sufficient clothing for their child’s daily needs and a spare set of clothing in case of accidents. Sun cream and sunhats should be supplied in the warmer months. Suitable footwear and clothing for outdoors must also be provided.
Accidents and illness
The nursery reserves the right to administer first aid and any emergency treatment as required. Parents will be informed of all accidents and will be asked to sign an Accident record form. If emergency treatment at hospital is required, the nursery will make all reasonable attempts to contact the parents but if this is not possible we are authorised to act on behalf of the parents and authorise any necessary emergency treatment.
Children who have, or develop, an infectious illness must be excluded from nursery for a minimum of 48 hours. This is in the best interest of the child and the other children and complies with regulations set out by the Environmental Health Department. The nursery must be notified of all absences.
Sickness or absence from Nursery does not qualify for a reduction in fees, as you must still pay for the child place at the nursery.
The nursery will administer prescribed medicines and Calpol, only if the parent(s) has given consent on our consent form.
Car Parking
There are designated parking bays for dropping off and collecting children. If these bays are full please ensure that you park your car carefully. Under no circumstances should any car be left parked in an unsuitable manner that could cause danger to children or to the general public.
Digital Images storage
In accordance with our storage of digital images policy, all images which have not been used to make an observation, will be deleted on a weekly basis. Any photographs which have been printed and used for displays, planning and promoting the provision, after they have fulfilled their purposes, will be used to compile an evidence folder or children’s folder.
EyLog
The nursery will be employing a comprehensive tablet PC and web-based method of carrying out observations, planning and assessments on your child, known as early years Log (eyLog). EyLog, will allow us to create electronic learning journeys using photographic, video and sound observations; and this will be able to be shared directly with you through email. So that only you can access your child’s content, the nursery manager will provide you with a unique secure pin number. We ask all parents and carers to ensure this pin number is kept confidential.
Agreement
These Terms and Conditions represent the entire agreement and understanding between the parents (or carers) and the nursery. Any other understandings, agreements, warranties, conditions, terms and representations, whether verbal or written, expressed or implied are excluded to the fullest extent permitted by law. We reserve the right to update / amend these Terms and Conditions at any time. One-month notice will be given of any changes made.
The nursery is operated by Fun Time Nursery Derby Limited.
